Inside this meticulously crafted binder, you will find neatly organized sectioned dividers that correspond to various referral specialties, as well as transitional services that are carefully aligned according to each client’s specific age and developmental stage, promoting clarity and ease of use. Each C.A.P is crafted based on the client’s primary disability, encompassing a curated selection of recommended referral resources that are tailored specifically to meet their unique needs, ensuring that all facets of support are covered. Additionally, three thoughtfully scheduled follow-up consultations within the year are included to ensure continuous support, guidance, and measurable progress over time, fostering a comprehensive approach to personal development and overall well-being that adapts as the client evolves.
